The category honours unsung heroes that demonstrate best practice within the home care sector, by mobilising and managing efficient teams that deliver outstanding end user support.
The nomination follows a year of phenomenal growth for CLC, with the firm on track to record a 117 per cent rise in annual turnover and demand for personalised support up a further 60 per cent compared to this time last year.
Chantel will now undergo a judging day with a panel of industry peers who will decide the overall winner for each category, to be announced at a gala dinner on Saturday 1 November at the Palace Hotel, Manchester.
If successful, Chantel will progress onto the national ceremony to be held early 2015.
Commenting on the news, Chantel said: “This achievement tops off a superb year for the team and I.
“As a home care co-ordinator it’s about having a love for what you do and a passion for helping others to achieve individual outcomes – that’s what fuels you every day.â€
Community Life Choices specialises in the provision of five-star home care to adults and children in need of personalised health and social care services.
Alan Frew, managing director at Community Life Choices, added: “Chantel has made significant impact in a short time. She is a much-loved, highly-valued member of the team who goes above and beyond the call of duty, always with a smile on her face, to ensure our clients receive the very best standard of care.â€
