We are looking for a committed care co-ordinator to join our team.
Summary of Post: To maintain continuity of care and support to Service Users and to ensure efficiency and reliability of all visits to Service Users.
Main Responsibilities
Assisting in the day to day business
Assisting with the day to day management of the care being delivered and ensuring that a high standard of care is being maintained
Organising and facilitating team meetings
Delivering medication and auditing medication on a weekly basis
Weekly spot checks/observations
Completing observations for care certificate
Ensure that all Service users records are kept up to date
Office duties which may include HR responsibilities
Out of office hours “On Call” as required
support in the community as and when required
Job Type: Full-Time
Required Experienced
Understanding of service provision in the care profession
Skills/Attributes

The successful applicant must have similar experience, will be an excellent communicator and a methodical worker able to ensure tasks are fully completed in a pressurised environment. Hours are 9am – 5pm Monday to Friday, although applicants must be flexible as the role will involve occasional work outside of these hours.
